The Sheridan School District is requesting applications from medical providers to partner with the Jacket Health and Wellness Center for the provision of school-based health services.
Completed applications are due to the Jacket Health and Wellness Center, 512 W. Church Street, Ste. A, Sheridan, AR 72150, on or before April 22, 2022. Minimum provider requirements, applications and instructions are available by clicking on this link.
The Jacket Health and Wellness Center provides access to on-campus basic physical and mental health services, as needed. The Center provides services beyond the scope of the school nurses and is not intended to replace the school nurse. The Jacket Health and Wellness Center acts as a resource center for wellness and prevention for public school students and staff.
