Flu Shot Clinic forms were sent home with students last week. If you did not receive the forms, they can be picked up in the SMS office or printed from the following link.
The SMS Flu Shot Clinic will be held on October 12, 2023. The FERPA and Flu Consent forms must be turned in on Monday, October 2, 2023. If you have any questions you can contact Nurse Sunni at 870-942-3813 or sunnimathews@sheridanschools.org.
